ANo. As long as you and your opponent(s) mutually understand which card you mean, there is no need to recite the exact name of the card.
Additionally, if you forget the name of a card during an official tournament or event, you may ask a judge for assistance in remembering, provided you abide by said tournament or event's rules.
QCan I declare the name of a token card, such as Fairy?
AYes, you can.
QCan I declare a collab card's alternate name like Lelouch, Leader of the Black Knights?
ANo. An alternate name isn't a card name. However, if you declare Demonlord Eachtar, then reveal Lelouch, you would draw 2 cards, since Lelouch has the card name Demonlord Eachtar.
QIf the card I reveal is not the card I declared, is the 1 card I draw the same card I revealed?
AYes, that's correct. If you declared correctly, the 2 cards you draw would be the revealed card and 1 more.
