AYes, it triggers even if Orchis itself leaves the field at the same time as a Puppet.

QIf I select a token for Paula, Passionate Warmth's On Evolve ability, does it get removed from the game?

ANo. It remains in its owner's EX area after you put it there. Follower and amulet tokens may exist in the EX area, field, and resolution zone. They are only removed from the game immediately after movement when moved to other zones.

ANo. Evolving a follower doesn't count as playing a card, so it doesn't count toward your Combo.
